Quote results shown on this site are illustrations based on the information submitted and are subject to carrier underwriting, inspection, and approval. Nothing on this site binds, alters, renews, or cancels coverage. No coverage exists until a licensed agent confirms it in writing and the carrier issues the policy.

Requests submitted through this site are received during business hours and are not a substitute for contacting your carrier or agent about urgent coverage needs.

Accuracy of information you provide

You agree that the information you submit is accurate, complete, and submitted with authority to act for the organization named. Material misrepresentation or omission on an insurance application can result in a denied claim, a rescinded policy, or a repriced premium.

US Church Insure is a brand of Provident Financial Group, a licensed independent insurance agency. NJ resident license; licensed in 21 states — NJ, NY, PA, NC, GA, FL, OH, TX, CA, CT, VT, MA, DE, MD, VA, SC, MI, KS, KY, AZ and NV. State license numbers available on request. We are an independent agency, not an insurer; coverage is underwritten by the issuing carrier and is subject to that policy's terms, conditions, limits and exclusions. Quotes are not offers of insurance, and no coverage is bound until confirmed in writing by a licensed agent. Carrier names are used for identification only; we are not affiliated with, endorsed by, or sponsored by any carrier named on this site, and no carrier endorses this agency.

These terms are governed by the laws of the State of New Jersey, without regard to its conflict-of-laws rules, and any dispute will be brought in the state or federal courts located in New Jersey. Insurance policies placed through our agency are governed by the law of the state in which they are issued, and mandatory consumer protections of your home state still apply.

We may revise these terms as our services or the law change. The revision date below reflects the most recent update, and continued use of the site after a change means you accept the revised terms.
